You need to fill out the application form and wait for it to be accepted. Once done you will be sent all the information.

If you have applied for membership through this form portal.php?getpage=application_form, you should recieve an e-mail with what you need within a few days.

afterhours, you have to post the avatar pic to the internet somewhere. If you find one you like, download it, open it in paint or some image program, resize, and post it back up on the internet. There's some sights aroung that will let you host images for free, including pics.blackdagger.net.
