I have a general question, which has recently come to my mind:
Are the enemies one has to face just representative for 'hordes' of them? I ask, because 'one screen', which one can pass in a few seconds stands for a few miles either, so it could be that the small numbers of foes are just for gameplay, engine, however.
Im just curious...
A question about quantities

Its not exactly quantifiable but the answer essentially is yes. This is certainly the case for NPC commoners and villagers etc. Zvidureth doesn't have a population of about 6 NPC's as we can see but has hundreds or thousands. Same goes for all the cities and towns. Given that I would assume the same for monster NPC's.
Pretty much what FM just said, but I think with "hordes" of monsters you wouldn't see people soloing certain areas...
It's pretty much use common sense based upon the area and the things in question.
For example, Zvidureth has an army barracks, so it's logical to think that hundreds of solders would be based there. But you don't see it in game, it's unfeasible for us to put 100 npc's in there. The engine isn't really designed to cope with so many NPCs in one place. (I think)
