I have also found the same but also with my weapon quickslots.

It seems to only happen when you store your quickslot item inside a bag(magical).

As Nightshade said I have just learned to adjust my quickslots on log in and server transition.

Been on for about 6 hours today, lots of crafting. I have seen none of the menu errors in the crafting tables that were present before.
