Was this ever completely fixed?

It does not appear so Deider. Prahksu Vernum is currently an Invalid (more so than usual).

It only affects you if your first three levels are not monk. The monk speed feat is re-applied on the third character level not the third monk level.

Just found this out the hard way. Made a note on the wiki entry that speed_toggle is bugged for multi-class monks if the 3rd character level that you took was not monk. Is that accurate?

It is the first three levels that have to be monk. Shouldn't matter beyond that.

Strange it was fixed in November of 2015, or at least we tested it in QA back then and the fix was approved and worked. Think it never went live which is strange.

It appears that the fix for this was created way back in 2013, but never imported into the modules. I'm digging up those files from the repository and will push them through as soon as possible.

In the meantime, don't use /speed_toggle if your first 3 levels weren't monk levels.

I'd advise not using it at all. The fixed version (a different command) records, and restores, the level you gained the feat at, which isn't always by your 3rd character level. Without that, the (current) live version may break more things, with picky checks for every level (and feats gained at them) being valid.

Resist the urge to be normal, and keep on speeding for now! :P
